Modern Treatments for Saphenous Vein Reflux

In the world of vascular health, few advancements have been as impactful as the evolution of treatments for saphenous vein reflux. Gone are the days when painful vein stripping was the only option. Continue reading to learn about the modern, minimally invasive techniques that have revolutionized the field.

What is Saphenous Vein Reflux?

Saphenous vein reflux is a medical condition where blood flows in the wrong direction within the saphenous vein, typically due to valve dysfunction.

The saphenous vein is a large vein running along the length of the leg, with two main types:

  • The great saphenous vein (GSV) runs along the inner thigh and calf.
  • The small saphenous vein (SSV) runs along the back of the lower leg.

Vein valves keep blood flowing upward toward the heart against gravity. When these valves become weak or damaged, they fail to close properly, allowing blood to flow backward (the “reflux”). This backward flow causes blood to pool in the lower leg, leading to symptoms such as:

  • Leg heaviness or fatigue
  • Swelling
  • Varicose veins
  • Pain or aching
  • Skin changes or ulcers in advanced cases

While not all saphenous vein reflux causes varicose veins right away, it’s often the underlying reason varicose veins appear. Even before anything is visible, you might feel discomfort or notice swelling.

Causes of Saphenous Vein Reflux

  • Genetics
  • Pregnancy
  • Prolonged standing
  • Obesity
  • History of blood clots

Treatments for Saphenous Vein Reflux

For decades, vein stripping was the go-to treatment. It involved general anesthesia, a hospital stay, significant post-operative pain and lengthy recovery times. Thankfully, this procedure has largely been replaced by more patient-friendly options.

Minimally Invasive Treatments

The last two decades have dramatically shifted how we approach saphenous vein reflux. Endovenous techniques have taken center stage, offering:

  • Minimally invasive procedures
  • Local anesthesia
  • Outpatient treatment
  • Faster recovery times
  • Excellent long-term results

At Vascular and Interventional Experts (VIE), we focus on two primary endovenous techniques:

1. Radiofrequency Ablation (RFA)

RFA has been our mainstay treatment for years, and for good reason:

  • Uses heat to seal off the problematic vein
  • Performed under local anesthesia
  • Typically, it takes about 45-60 minutes
  • Patients usually return to normal activities within a few days

2. VenaSeal

VenaSeal is newer and effective:

  • Uses a medical adhesive to seal the vein
  • There is no need for multiple needle sticks or tumescent anesthesia
  • Often shorter procedure time compared to RFA
  • Patients can return to normal activities almost immediately
  • Less post-procedure pain and bruising

VenaSeal’s performance:

  1. Reduced Discomfort: Significantly less pain during and after the procedure
  2. Rapid Recovery: Patients often resume normal activities the same day
  3. No Compression Stockings: Unlike other treatments, post-procedure compression isn’t necessary
  4. Precision: Targets only the problematic vein, minimizing the impact on surrounding tissue

Laser Treatment

At VIE, laser treatment was once popular, but we’ve moved away from using laser for saphenous vein reflux. In our experience, compared to RFA and VenaSeal, laser treatment tends to cause more pain, increased bruising and longer recovery times. Our goal is to provide the most effective treatment with the least discomfort, so we focus on RFA and VenaSeal.

Choosing Your Vein Treatment

The best treatment for you depends on several factors:

  • The size and location of your veins
  • Your overall health
  • Your lifestyle and recovery needs
  • Insurance coverage (VenaSeal may not be covered by all plans)

Both RFA and VenaSeal offer significant advantages over traditional vein stripping, including:

  • Minimally invasive approach
  • Use of local anesthesia
  • Quick procedures (often less than an hour)
  • Faster recovery times
  • Excellent long-term results
